1. Pursuant to the provisions of reference (a), Petitioner, a
former enlisted member of the United States Navy, filed
enclosure (1) with this Board requesting, in effect, that the
RE-4 (not recommended for retention) reentry code issued on 2
September 2004, be upgraded.

2. The Board, consisting of Mr. Zsalman, Ms. Polk, and Mr. Tew,
reviewed Petitioner's allegations of error and injustice on 30
July 2014, and, pursuant to its regulations, determined that the
corrective action indicated below should be taken on the
available evidence of record. Documentary material considered
by the Board consisted of the enclosures, naval records, and
applicable statutes, regulations and policies.

3. The Board, having reviewed ail the facts of record
pertaining to Petitioner's allegations of error and injustice

finds as follows:

a. Petitioner enlisted on 5 August 2004. She had no
disciplinary action during her period of service. She was
diagnosed with an adjustment disorder with depressed mood and
administratively separated. She received an uncharacterized
entry level separation on 2 September 2004, and was assigned an
RE-4 reentry code. She could have been assigned a waivable RE-
3G (condition, not physical disability, interfering with

‘performance of duty) reentry code.
CONCLUSION:

Upon review and consideration of all the evidence of record, the
Board determined that as a matter of clemency, Petitioner’s RE-4
reentry code should be changed to RE-3G. In view of the above,
the Board directs the following corrective action.

RECOMMENDATION:

a. That Petitioner's naval record be corrected to show that
on 2 September 2004, she was assigned an RE-3G reentry code vice
the RE-4 reentry code.

b. That a copy of this report of proceedings be filed in her
record.

4. Pursuant to Section 6(c) of the revised Procedures of the
Board for Correction of Naval Records (32 Code of Federal
Regulations, Section 723.6(c) it is certified that a quorum was
present at the Board’s review and deliberations, and that the
foregoing is a true and complete record of the Board’s
proceedings in the above entitled matter.

Naval Records (32 Code of Federal Regulation, Section 723.6(e))
